Please complete the following form to request short-term absences such as medical appointments, visits to Secondary Schools and one-off events which occur during a single day.
If you are considering requesting school absence from the Head Teacher for a more than a day or for a holiday during school term, this request must be made in writing to the Head with reasonable notice.
Events such as holidays will be marked as unauthorised in the school register, unless there is a significant pastoral reason for the time away from school, decided at the Head’s discretion. More information on requesting absences is available in the school handbook.
